Instructions For Form CFS-1
Please type information in designated boxes.
Mail original Form CFS-1 and CFS-1 Supplemental Sheet if needed to:

Check or money order must be included with all filings. Make check or money order payable to State of Utah.
Copies attesting to filing will be returned to secured party, except in the case of an assignee. Documents presented after 4:00 p.m. will be dated and processed the following business day.1) and 2) Give name, address and social security number, or if other than a natural person, the IRS taxpayer identification number of each person subjecting the farm product to the security interest. If additional boxes are needed, use Form CFS-1 Supplemental Sheet. If individual, give last name first. For an assumed business name of a sole proprietor, his/her name and social security number must be entered. If multiple individuals are involved, their names and social security numbers must be given. For other types of business, list business name as registered with the Division of Corporations for the State of Utah.
3) Give name and complete mailing address of the secured party.
4) Give name and complete address of the assignee (if any).
5) List each specific farm product description by name as listed below (no general or miscellaneous categories will be accepted). List each county in the State of Utah where the farm product is produced or to be produced at the time this EFS is executed. If in more than one county list each county. List the crop year unless every crop of the farm product in question, for the duration of the EFS, is to be subject to the particular security interest. The Utah State Central Filing System will interpret the term "CROP YEAR" as follows:
a) For a crop grown in soil, the calendar year in which it is harvested or to be harvested.
b) For animals, the calendar year in which they are born or acquired.
c) For poultry or eggs, the calendar year in which they are sold or to be sold.Any EFS filing not showing the crop year will be regarded as applicable to the crop or product in question for every year for five years from the date of filing subject to extension for additional five year periods.
6) Signature(s) of persons subjecting the farm product(s) to the security interest. If more than two additional persons will sign on form CFS-1 Supplemental Sheet.
7) Signature of authorized representative of the secured party.8) Complete name and address of person to whom the stamped copy should be returned (lawyer, assignee, etc).
