Instructions for Form CFS-3
Please type information in designated areas.
Copies attesting to filing will be returned to secured party, except in the case of an assignee, in which event copies will be returned to assignee. Documents presented after 4:00 p.m. will be dated and processed the following business day. Any attachments must be submitted in duplicate. If additional space is needed, use a second form CFS-3.1) Filer must indicate which type of supplemental filing this is. Amendments must be signed by all debtor(s) and secured party(s). In cases of Termination or Continuation Amendments only the secured party(s) need sign.
2) Give the original EFS filing number.
3) and 4) Give name, address and social security number or if other than a natural person, the IRS taxpayer identification number of each person subjecting the farm product to the security interest. If individual, give last name first. If other than a natural person, list business name as registered with Division of Corporations for the State of Utah.
5) Give name and complete mailing address of the secured party.
6) Give the original EFS filing date.
7) Give specific instructions as related to the supplemental filing.
