Ordering/Obtaining Copies & Certificates

All requests for Certificates of Existence, certified copies, copies of records, or archive searches must be made in writing.

The Division cannot accept orders or payments via telephone or fax.

Division processing times for these types of orders take 3-10 working days to process and deliver.

An expedite service is available for an additional $75 per document for a 1-2 business day process time.

Obtaining Plain Copies of Filed Documents

You can obtain plain copies of previously filed documents online by doing a Business Entity Record Search.  Currently, this service is free.  However, fees ranging from $1-$3 to search for business entity principals will return (see fee schedule) once the new platform is fully built.  To obtain copies of publicly available records:

  • Go to businessregistration.utah.gov  , opens in a new tab.
  • Below the login button, click on Search Business Entity Records.
  • Search by entity name OR entity number.
  • Click on the name of the entity to view the record.
  • Click on Filing History at the bottom of the screen to view filed documents.
  • Under View, click on the magnifying glass next to download the document.

Purchase a Certificate of Existence

 
Fees
  • Regular Certificate of Existence: $12.00 (Most common type ordered. A one page document that indicates the existence of the business entity and its status.)
  • Long Form Certificate of Existence: $20.00 (Indicates the existence of the business entity, its status, any restated articles, and any amendments to date.)
  • Expedited Fee: An additional $75 (Processing time is up to 2 business days).
How to Order a Certificate of Existence

This step applies for all entity types except DBAs (scroll down for DBAs). Please note, a Certificate of Existence cannot be purchased for entities that are dissolved, expired, revoked, or cancelled. A Certificate of Existence can be purchased for entities that are Current or Delinquent.

    • Once complete, submit the order from via mail, in person, or by email to [email protected].
    • Payment options:
      • ***DO NOT SEND PAYMENT INFO BY EMAIL OR FAX. THE ORDER WILL BE REJECTED***
      • Mail the order with a $20 check to the Utah Division of Corporations at P.O. BOX 146705, Salt Lake City, UT 84114-6705.
      • Drop off the order and make payment in-person at the front counter at 160 E. 300 S., 1st Floor, Salt Lake City, UT.
      • Email the order to [email protected]. When your order is ready, we will email you a link to pay online.
How to Order Certificate Registration for a DBA

A DBA Certificate of Existence is actually a Certificate of Registration and cannot be purchased through the new online platform (yet).

  • Submitter's Information (name, address, phone # and email)
  • Service Requested (mark whether Short Form Certificate or the Long Form Certificate is needed)
  • Entity Number
  • Business Name
Notarized Documents

If the certificate is going outside the United States, it may need to be authenticated by the Lt. Governor's office. The order form has a box to check mark and a line to enter the country name, if it needs to be notarized for an apostille. Refer to authentications.utah.gov for information on an apostille.

  • Certified: $12.00 for the certified stamp plus $.30 cents per copied page.
  • Non-Certified: $0.30 cents per copied page.
  • Expedited Fee: An additional $75 (Processing time is up to 2 business days).
