Ordering/Obtaining Copies & Certificates

Because of difficulties in meeting the needs of customers requesting information from the Utah Division of Corporation's repository, the Division is implementing new procedures for those requesting Division documents.

All requests for certified copies, Certificates of Existence, copies of records, or archive searches must be made in writing by mail, fax, or in person. Phone requests will not be accepted.

The Division will accept requests for corporate archive searches, Certificates of Existence, certified copies and copies of records by mail at: 160 East 300 South, Box 146705, Salt Lake City, Utah 84114-6705; via fax at: (801) 530-6438; or in person at the Division offices at: 160 East 300 South, 2nd Floor, Salt Lake City. Generally, requests for information from the Division will take 3-10 working days to process and deliver. An expedite service is available for an additional $75 per document for a 1-2 business day process time.

Depending on the type of information requested there is a fee ranging from a small charge for copies, to a $12.00 fee for Certified copies and Certificates of Existence.


Obtaining Plain Copies of Filed Documents

You can obtain plain copies of previously filed documents online by doing a Business Entity Record Search.

  • Go to businessregistration.utah.gov .
  • Click on Search Business Entity Records.
  • Search by entity name OR entity number.
  • Click on the name of the entity.
  • Click on Filing History at the bottom of the screen.
  • Click on the spyglass next to the document you want to download.

Certificate of Existence

For all entity types except DBAs:

  • Log in to the system linked in the red button above using your UtahID (If you do not have a UtahID, click here to create one). 
  • Select Copies and Certificates from the left side menu
  • Select Request Certificate of Existence

How to Order a DBA Certificate of Existence/Registration

DBA Certificate of Existence is a Certificate of Registration. You can purchase one by filling out the Order Form , selecting the Certificate of Existence option, and submitting online via the Digital Marketplace.

Fee

Short Form - $12.00 (One page document that indicates that the business entity exists and whether it is current)
Long Form-$20.00 (indicates the existence of the business entity, whether it is current, and all restated articles, amendments to date if any)

  • Note: Long Form must be requested via the Order Form and submitted to our office.

A Certificate of Existence cannot be ordered if the status of the record is:

  • Involuntarily/Voluntarily Dissolved
  • Expired
  • Revoked
  • Cancelled

A Certificate of Existence can be ordered if the status of the record is:

  • Current
  • Delinquent

Required Information on Order Form when Requesting a Certificate of Existence via in-house submission:

  • Pickup or Mail
  • Expedite or Regular Service
  • Submitter's Information ( Name, Address, Phone # and Email)
  • Service Requested (mark whether Short Form Certificate or the Long Form Certificate is needed)
  • Entity Number
  • Business Name

Notarized Documents

If the certificate is going outside the USA, it may need to be authenticated by the Lt. Governor's office. On the Order Form , there is a box to check mark and a line to put the country if it needs to be notarized for an apostille. Refer to authentications.utah.gov  for information on an apostille.

You can order a Short Form Certificate of Existence for all entities, except DBAs, completely online at businessregistration.utah.gov .

If your order requires notarization to be apostilled, submit it online via the Digital Marketplace.

When ordering the Long Form Certificate of Existence, submit the Order Form via fax, mail, or drop off in-person with payment/payment info.

Fee

$12.00 for the certified stamp (Certified) plus $.30 cents per copied page
$.30 cents per copied page (non-Certified)
