$12.00 per filing. You can send it by mail or bring it in person to 160 east 300 south, p.o. box number 146705. Salt Lake City UT 84114-6705. We do not accept filings by fax.

How do I pay for the filing? (Click for Details)

You can pay by check & make it payable to the state of Utah. You can use visa/master card or american express. You must list expiration date for the card.

How do I know who needs to sign on the documents? (Click for Details)

On a CFS 1 filing all parties must sign. On continuations & terminations, only the secured party needs to sign. On amendments changing all other information, signatures are required from all parties. On all amendment actions of a CFS filing the active debtors/secured parties must be listed with a complete address & social security/tax ID numbers.

How are CFS filings & UCC filings different? (Click for Details)

CFS filing is a listing of information to identify farm products that are subject to a security interest in Utah. A UCC filing perfects the lien.

How to fill out a CFS 1 (original) form? (Click for Details)

All forms submitted must be typed or computer generated. All individual debtors listed must have a complete address & list social security numbers. All business name debtors need to list a complete address & list tax ID numbers, all secured parties must list a name and complete address . You must specify collateral & list the Utah County where the product is produced. All parties must sign.

CFS filings are amended using the Statement of Continuation, Termination or Amendment form.
